Part B explores my practice, The Community Palliative Care Network (CPCN), created to provide community care in the Ottawa area for patients at near-end-of-life. I started my practice as a solo physician, evolving to a supportive group of physicians, coordinating our care with the network of resources for community palliative care already established in the Ottawa area. My vision was to investigate the feasibility and sustainability of delivering near-end-of-life care in patients’ homes. The set-up of my practice, the goals, the successes, the failures, and what I learned along the way are documented. Patients’ journeys provide examples of authentic moments of building an understanding of the science and art of near-end-of-life care.
Reflections of lessons learned along the way are the building blocks for my emerging philosophy related to near-end-of-life care.
