This journal was written specifically to prove to my patients and families that through their journey, they had made a significant contribution to help future patients and families with their journey at near-end-of-life. Part E presents the summary of their journey by specific diagnoses, combined into groups with similarities in their paths toward death. I reviewed, analyzed, and compiled all my patients’ information to demonstrate these likely paths toward death for each specific diagnosis. Within each group, I include a patient and family’s journey to highlight a real-life example of the fusion of the science with their art of care at near-end-of-life.
