PART C

Part C explores the creation and philosophy of my new Near-End-of-Life practice after closing my CPCN practice. I returned to providing care to a smaller group of patients asa solo physician. I set up my practice incorporating all my experiences and the tools I had developed. My vision was to fully engage patients and families in their care. I explained the science of near-end-of-life to them. I involved them fully in the decision-making process. I encouraged them to fuse their art of care to the science to optimize their unique journey toward death.
